Appointments

For an appointment, please call the number for the doctor whom you would like to see. If circumstances prevent you from keeping an appointment or you anticipate being late, please notify our office and we will be happy to reschedule.

Please verify participation in your insurance when making your initial appointment. Patients are responsible for obtaining referrals from their primary care physician.

Surgery Scheduling

Should you require surgery the staff will make every effort to see that your insurance company has been notified and that all precertification requirements have been met.

You are our best source of information regarding your coverage. Please provide us with up-to-date information regarding all of your insurance policies.

Office Policies

Insurance
We offer professional assistance in filing your insurance. Our physicians accept most major insurance plans including Medicare, Aetna, Blue Cross and many preferred provider insurance plans.

Prescription Policy
Prescriptions are refilled on Monday thru Friday between the hours of 9:00 a.m. and 4:30 p.m. only. Prescriptions will not be refilled after hours or on weekends. Please anticipate your needs and call early.

Billing Procedures
Office visits: Payment at time of service is required for all patients except Medicare, Medicaid or one of our contracted managed care plans.

Surgery: A deposit is required for surgical procedures if you have out-of-pocket expenses greater than $250.00. Payment plans can be arranged prior to surgery.
